Summary
Healthcare professionals are facing a historic volume of patients – leading to increased workloads and complex patient journeys that are challenging to manage. Partnerships that encourage communication and data-sharing across interdisciplinary healthcare professionals delivers improved patient outcomes by reducing clinical burden for providers, among other benefits. In a session designed to effectively illustrate the power of partnerships, you will hear from evolvedMD, a leading behavioral health services integration company, and NeuroFlow, a best-in-class technology and care services company for integration of behavioral health, leadership on how healthcare services and technology can come together to provide meaningful and effective solutions to both patients and behavioral health providers. Through this innovative collaboration, evolvedMD behavioral health providers utilize NeuroFlow’s cloud-based registry to enable real-time collaboration and management of patient panels with straightforward time-tracking and documentation, helping to scale psychiatric collaborative care (CoCM) at 24-primary care clinics throughout the southwest. Bridging and continuing care between appointments, patients have access to NeuroFlow, a behavioral application offering evidence-based resources and tools to support overall health. This partnership brings together high-tech and high-touch care, providing patients with proactive, personalized support while simplifying clinical workflows. Moreover, evolvedMD also offers NeuroFlow to their employees to support mental and behavioral health, and to provide behavioral health providers the opportunity to personally experience the positive impact of technology-enabled integrated care. In this session, we will provide an overview of evolvedMD and NeuroFlow and detail how this partnership has overcome specific challenges of integrated behavioral health (10 min). Next, we will highlight behavioral health providers’ experiences of utilizing NeuroFlow as a tool in the care they give to patients, and as a personal wellness tool (5 min). Lastly, we will identify how other integrated care settings can form a similar partnership (5 min). The remaining time will be Q&A (5 min).
